What Information We Collect
To provide you with high‑quality diagnostic imaging and women’s health care, our doctors, counsellors, and sonographers need relevant information about your past and present health.
Your discussions with our medical staff are private, and any information that can identify you and relates to your medical history or care is known as personal health information.
Our records may include details such as your:

  • Name, address, date of birth, and contact details
  • Medicare and/or private health insurance number
  • Medical history, clinical images, pathology results, and referral details
  • Blood group or relevant laboratory results

All personal and medical information is kept private and securely stored.

Your Medical Records
Our doctors, counsellors, and sonographers take great care to ensure that your medical records:

  • Are accurate, comprehensive, and up to date
  • Contain sufficient information for another healthcare provider to care for you if required
  • Do not include irrelevant or inappropriate comments
  • Include a concise summary of your care and treatment

We only collect information that is directly relevant to your medical care. If you are ever unsure why certain information is being requested, please ask your doctor or sonographer.

Providing Your Information to Others
Your personal health information is used primarily to assist in the accurate assessment, diagnosis, and reporting of your ultrasound or diagnostic procedure. This may involve providing reports to your referring doctor or sending samples to pathology or diagnostic laboratories.
By consenting to testing or treatment, you also consent to relevant information being provided to these supporting services if required for your clinical care.

It is standard practice for authorised clinical staff within Waves Ultrasound for Women to have access to medical records for the purpose of providing comprehensive care.

Your personal health information will not be disclosed to any external party unless:

  • You have given consent; or
  • It is required to prevent a serious threat to your health and you are unable to give consent; or
  • It is required by law (e.g., notification of certain infectious diseases, court order, or subpoena); or
  • It is necessary to obtain Medicare or private health fund payments.

Only the minimum information required for these purposes will be shared.

Use of Health Information for Quality Improvement and Research
We may use health information (in a de‑identified form) to help improve the quality of care we provide across our practice, such as through service audits and quality reviews.

We may also contribute de‑identified data to medical research or educational publications that aim to improve community health.
Your personal identity will never be disclosed in any published material or research findings.
